Criteria for Application to Sports Medicine Fellowship

All applicants must pass the allopathic and/or osteopathic boards on the first attempt prior to beginning the fellowship. For those applicants who do not pass on their first attempt, their files are placed on hold for special review by the program director.

For international graduates, they must pass the USMLE parts I and II on the first attempt, with passing scores of 80 or above on both parts, or with an average passing score of 80 or above. They must also pass the CSA examination and the English examination on the first attempts. They must have ECFMG certification, and be graduated from medical school within five years. Documentation of CME hours for each of the years following graduation from residency must also be submitted.

All applicants must have a good command of the spoken English language.

All applicants must be US citizens or permanent residents of the US. We do not sponsor visas.

All applications must be complete with the following:

• ECFMG certificate (if applicable)
  
• Board scores
  
• Dean's letter (if applicable)
  
• Illinois license (current)
  

Applicants must be successful graduates of a residency program in Family Medicine, Internal Medicine, Pediatrics, Emergency Medicine, or Physical Medicine & Rehabilitation.
